New Official Prescription Forms
Effective September 1, 2018, the Texas State Board of Pharmacy will begin to issue new Official Prescription forms to Texas licensed practitioners. The changes are being implemented to provide several advanced security features to address increasing problems with fraudulent forms.
One of the highest levels of document security has been added to include a true watermark, which will be pressed into the paper and is visible from either side in regular light. This feature resists photocopying and scanning.
Additionally, the thermochromic ink, currently on the back as a red check, is being moved to the front of the form as the “Rx” symbol. This feature can be verified by rubbing the red “Rx” symbol with the fingers, at which time it should fade and then reappear.
Current security features are still present, such as the void watermark and chemical reactivity.
PLEASE NOTE: Effective January 1, 2019, only the new Official Prescription Form will be accepted and all other versions of the Official Prescription Form will be considered invalid.
