At the January 17, 2018 meeting of the Iowa Board of Pharmacy (board), the following public actions were taken during open session:
Connor Wachter, Technician, agreed to surrender his technician registration.
Alicia Diaz, Technician, agreed to surrender her technician registration.
Stephanie Carberry, Technician, entered into a Settlement Agreement and Final Order to voluntarily surrender her technician registration.
Anthony Allee, Pharmacist, entered into a Settlement Agreement and Final Order with the board. The board issued a citation and warning and imposed a civil penalty of $1,000 for violating certain duties of a pharmacist in charge. The board further ordered completion of additional continuing education.
Leeds Pharmacy entered into a Settlement Agreement and Final Order with the board. The board ordered the pharmacy be placed on probation for a period of 2 years with conditions. The board also imposed a civil penalty of $1,000.
Robert Rehal, Pharmacist, entered into a Settlement Agreement and Final Order with the board. The board issued a citation and warning and imposed a civil penalty of $500 for violating the duties of a pharmacist in charge and pharmacy owner of Leeds Pharmacy. The board further ordered completion of additional continuing education.
United Specialty Pharmacy entered into a Settlement Agreement and Final Order with the board. The board issued a citation and warning and imposed a civil penalty of $2,000 for failure to perform/ensure complete drug use review and for utilizing an unlicensed central processing pharmacy.
NuCara Compounding Pharmacy entered into a Combined Statement of Charges, Settlement Agreement, and Final Order with the board. The board issued a citation and warning for having a license revoked or suspended or having other disciplinary action taken by a licensing authority of another state for conduct substantially equivalent to any of the grounds for disciplinary action in Iowa. The board furthered ordered a civil penalty in the amount of $2,000.
Ric Foreman, Pharmacist, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order with the board. The board ordered the pharmacist’s license be placed on probation for 3 years with conditions and pay a civil penalty of $2,500 for violating the duties of a pharmacist in charge and pharmacy owner at Evansdale Pharmacy.
Evansdale Pharmacy entered into a Combined Statement of Charges, Order to Show Cause, Settlement Agreement, and Final Order with the board. The Order suspended the pharmacy’s license, but the suspension was stayed for 60 days for correction of deficiencies. Following the suspension, the pharmacy’s license will be placed on probation for a period of 3 years with conditions. The board further ordered a civil penalty of $2,500.
Robert Osborn, Pharmacist, agreed to place his pharmacist license on inactive status.
Matthews Veterinary Pharmacy entered into a Licensure by Consent Agreement with the board. The board imposed a civil penalty of $1,000 for delivering prescriptions into Iowa without a license in 2017.
Richard Grote, Pharmacist. Following a hearing, the board ordered the suspension of his pharmacist license and a $5,000 civil penalty for failing to ensure legal operation, failing to maintain policies and procedures, failing to maintain a CQI program, and Failing to maintain records for Schedule V dispensing.
Premier Distribution Inc, Wholesale Drug Applicant. Following a hearing, the board denied the Wholesale Drug Application.
