(PHY.006003): For its failure to acquire a new phar-
macy permit when the ownership changed in January
2013, and for its continued operation of a pharmacy
without a valid pharmacy permit, and for its continued
dispensing of approximately 500 prescriptions into
Louisiana while its permit had expired during the
month of January 2015, the Board assessed a fine of
