The 2015 - 2016 renewal season is closed!
The renewal period for the following individuals and facilities closed as of April 1, 2015:
- Pharmacy Technicians
- Charitable Clinic Pharmacy Technicians
- Charitable Clinics
- Supplier of Medical Equipment, Legend Devices, and Medical Gas (DME license)
- Hospital Pharmaceutical Services
- Institutional Pharmaceutical Services
- Wholesale Distributor of List I Chemicals
- Wholesale Distributor of Legend Drugs
Renewal information for these individuals and facilities was sent out in October 2014 to the mailing address that we have on file. If you did not renew your license by April 1, 2015, your license is void.
Continuing to practice or operate in the state of Arkansas without proper licensure is illegal.
