Risk is a probability--in most cases, risks associated with off-label uses have not been systematically defined or measured.
If an overriding public health goal is to limit population exposures to therapies that are not systematically studied and vetted by multidisciplinary experts, then off-label promotion and use to achieve personal or corporate goals of medical communities should be viewed no differently than off-label promotion by drug companies. In the former, patients may be dangerously commoditized and mislead by merchants masquerading in white coats.
