Prior authorization provides a responsible mechanism for medically necessary compounds to be covered, at the same time weeding out unnecessary exposures to drugs that may be unsafe or ineffective.
Pass thru coverage for unapproved drugs without prior authorization carries an added burden of risk for beneficiaries--that risk should be actively accessed and judged "tolerable" given alternative therapies, anticipated benefits, medical necessity and urgency.
