Note one case involving a compounding error:


Docket Number 13-438            
Attorney Steven Schwane presented this case.  Commissioner Inguanti ‘stepped
down’.  This case involved pharmacist Cynthia Boccuzzi was involved with a
compounding prescription error involving a child.  The agreement calls for a one
year probation, two continuing education requirements, a Letter of Reprimand
and a $1,000 civil penalty.  
Commission Action:  Commissioner Carbray moved seconded by
Commissioner DeFazio and passed by a vote of 5-0 to accept and
approve the aforementioned settlement agreement.
