Reminder: FVS Holdings, Inc (dba Green Valley Drugs) is a perfect example for consumers of compounds whether it be doctors, dentists, vets, or patients that you must not only know the name of the pharmacy you are doing business with but also any other names that it may be listed under when researching that pharmacy.

Even on the FDA compounding webpage FVS and Green Valley are not linked together--although they are clearly the same.  See below.  So when researching especially disciplinary records make sure you search under both names.  Likewise, if you are searching under a pharmacist's name, make sure that you check under any variations of the names.  For example, if a female was married previously her name may be listed in one state on that name but in another state under her name from her second marriage.  Also check because while the name should be the official name of the person, it is possible that someone is listed by a nickname.
